The UK has been given an unexpected boost by the news that its erectile dysfunction drug, Cialis, has now been linked to a 20% increase in the risk of heart attacks and stroke.

Cialis is already available in the UK and can now be bought over the counter in some countries as a daily treatment. The new warnings are based on observational studies that have not been fully analysed in the UK, the UK's largest healthcare provider, and data from 2023.

"The risk of heart attacks and stroke was slightly higher with daily Cialis treatment, with a greater risk for those with existing cardiovascular disease than those taking a placebo," said Dr David Edwards, medical director of the Royal College of GPs. "However, the absolute increase in the risk of these events was not statistically significant."

The UK's National Institute for Health and Care Excellence (NICE) says it believes the new warning applies to all patients taking the medicine.

It says: "The results of the study support further research and, given the current evidence that Cialis does not increase the risk of heart attacks and strokes, we are confident that further information is available at this stage."

Dr Edwards added: "In the UK there is no evidence to show that daily Cialis increases the risk of cardiovascular disease or stroke, and we continue to monitor these potential risk factors."

The UK is still considering the safety of Cialis, but hopes it will be approved sooner rather than later.

In March the drug was approved for treatment of erectile dysfunction by the NHS in England.

Professor David Edwards, professor of urology at the University of Nottingham and former chief clinical officer of the National Institute of Clinical Excellence.

Professor Edwards says: "We really hope that Cialis will be an effective treatment for many men who are struggling to get and keep an erection.

"While it has been previously reported that daily use of Cialis increased the risk of heart attacks and strokes, and a meta-analysis of observational studies, we do not think there is a clear causal link between daily use and increased risk of these events."

Dr Edwards said: "The findings of this study suggest that daily use of Cialis does not lead to an increase in the risk of cardiovascular events in the majority of men, and that further research is needed to confirm these results."

Dr Edwards added: "This is not an isolated study, but we are very confident that further research is needed."

Cialis is currently available as a daily pill, but the safety and efficacy of daily Cialis has not been studied in the UK. The manufacturer has said it was reviewing other treatments for erectile dysfunction.

Dr Edwards added: "As part of our ongoing research to help ensure that Cialis is as safe and effective as possible, we are also reviewing other treatments that may be suitable for men with erectile dysfunction."

A review of the evidence concluded that the safety of daily Cialis was uncertain because of the long term safety of daily use.

The product licence for daily Cialis for sale in the UK has expired and has now been subject to a review.

In September the manufacturer was informed that it was reviewing a study that compared the safety of daily Cialis to a placebo and also concluded that there was not enough evidence that the combination was superior to the placebo. The company said this review will be continuing.

Dr Edwards added: "As part of our ongoing research and assessment of the safety of daily Cialis, we are reviewing additional studies to confirm these findings."

A recent study published in the British Journal of Urology showed that daily use of tadalafil, a daily Cialis product, increased the risk of a heart attack compared to a placebo.

The study, which was funded by Pfizer, found that people taking daily Cialis were three times more likely to have a heart attack compared to people taking a placebo. The researchers also found that people who took daily Cialis had a slightly higher risk of stroke than those who took a placebo.

Dr Edwards said: "In the first 2 years of clinical trials, the findings were similar in type and severity of erectile dysfunction, but the men who took the daily version were more likely to have a stroke, compared to those taking a placebo.

"As a result, the findings from this trial are now being confirmed in further studies to confirm these results."

A new warning was added to the product licence after this study was published in the British Medical Journal.

Whether you can take Tadalafil at the same time as your other medications will depend on which medications you are taking.
